Toni Abrams Weintraub

Toni Abrams Weintraub is a physician-researcher at Mathematica with more than two decades of experience in public and community health, health promotion, and health policy research. She specializes in social determinants of health, linkages between clinical care and social services, and community-based and cross-sector health initiatives, with a focus on underserved communities and individuals with mental health and substance use challenges.

Abrams Weintraub is currently the principal investigator for Mathematica’s implementation, learning system, technical assistance and monitoring contract for the Center for Medicare & Medicaid Innovation (CMMI) Accountable Health Communities Model. In this role, she supports the implementation of a CMMI model that integrates clinical care with community support services to address health-related social needs through screening, referral, and community service navigation. In addition, she is a task lead for the qualitative component of the cross-site evaluation of Boston Children’s Hospital Collaboration for Community Health grant program to improve the health and well-being of children and families, particularly those most affected by inequities in health and health care. She also serves as a subject matter expert to the prevention and health improvement team on the quality improvement contract for the Center for Medicaid and CHIP Services Core Set measures.

Before joining Mathematica in 2017, Abrams Weintraub spent over two decades conducting community health needs assessments, designing and implementing community-based health promotion programs, and evaluating community health interventions for nonprofit, private, and government health care organizations. Abrams Weintraub established and directed TAW Consulting and held positions at Health Resources in Action and Tufts Health Plan, among others. She directed the Preventive Medicine and Public Health Residency and the Community-Oriented Primary Care Program at Carney Hospital in Boston. She holds an M.P.H. from Boston University and an M.D. from Cornell University Medical College.
