The Money Follows the Person (MFP) demonstration has helped states support community living opportunities for people with disabilities who are eligible for Medicaid since 2007. This brief is designed to guide MFP grantees in developing a plan for using data to improve MFP program performance. This brief follows the Plan-Do-Study-Act (PDSA) quality improvement cycle as it guides readers through a process to close gaps between MFP program performance and program goals.
