This report summarizes a comprehensive evaluation of Healthy San Francisco, a program for uninsured adults ages 18 to 64. The evaluation found that program participants regularly receive outpatient care at their medical homes, including recommended preventive services; are using fewer emergency department services; and appreciate improved access to primary care and less uncertainty in meeting their health care needs. In general, providers also expressed satisfaction with the program and intended to continue participating.
