This resource describes how community coalitions, particularly those focused on healthy eating and physical activity, can participate in or align with community health assessments (CHAs) and community health improvement plans (CHIPs). CHAs and CHIPs are processes that identify local health priorities and inform investments. They often highlight issues these coalitions focus on, such as food insecurity, chronic disease prevention, physical activity, and access to healthy food. Coalitions can engage in CHA and CHIP processes to strengthen partnerships, align their strategies with community priorities, and support long-term sustainability and impact.
