Several states across the country are piloting an innovative total cost of care model to curb rising healthcare costs, strengthen primary care, and improve community health. Through CMS’s Achieving Healthcare Efficiency through Accountable Design (AHEAD) Model these participating states are testing new approaches to transform how care is delivered and financed.

Mathematica brings a strong foundation of experience to help states navigate these efforts. We’ve partnered with states, federal agencies, payers, and providers to implement models such as the Vermont All-Payer Accountable Care Organization Model, the Maryland Total Cost of Care Model, and the Pennsylvania Rural Health Model.

Here are a few examples of how we’ve supported states in this work:

  • Global budgets: Developing and implementing hospital global budgets and other population-based alternative payment models in states including Pennsylvania, Maryland, and Vermont
  • Multi-payer alignment: Achieving multi-payer alignment for alternative payment models through data analyses and collaborating with providers, payers, and model administrators
  • State accountability and data infrastructure: Assisting states, including Massachusetts, New Jersey, Connecticut, and California, with cost growth benchmark programs and exploring health care cost drivers through an interactive tool
  • Methodology development: Working with states to develop and refine the methodology of alternative payment and delivery models to align incentives with state and model goals; helping states use data to make informed decisions
  • Primary care: Evaluating state-based and federal total cost of care and primary care models and partnering to improve the delivery of primary care
  • Medicaid and Medicare: Working with states and CMS on Medicaid policy innovation, assessing state waivers, and evaluating Medicare primary care and value-based care models.
  • Recruitment: Recruiting providers and payers to participate in voluntary alternative payment models by producing financial models for informed decision making
