Key Findings
  • During a breakout session, attendees shared strategies for placing financial incentives on AHC Model activities, shifting organization culture to increasingly focus on addressing health related social needs (HRSNs), and leveraging partnerships to sustain AHC Model activities.
  • Dr. Donald Berwick, the keynote speaker, discussed health care access and population health disparities and the value of social determinants of health (SDOH) in addressing these gaps. His talk concluded with mention of how the AHC Model is shifting communities’ focus to addressing disparities in SDOH.

On February 3-4, 2021, CMS held the virtual AHC meeting “Planning for Sustainability.” During this meeting bridge organizations shared strategies for optimizing the AHC Model and sustaining the Model beyond the cooperative agreement period. As a response to the public health emergency and a national call for racial justice, this meeting also highlighted the importance of health equity in maintaining the AHC Model.
