  • According to Accountable Health Communities program data, food insecurity was the most common health-related social need among Medicare and Medicaid patients screened during the model.
  • According to Accountable Health Communities program data, community navigation services helped resolve over one-third of navigation cases.

This resource provides updated data and information on AHC Model recipients and their HRSN screening, referral, and navigation efforts. The findings in this document are based on data from the 31 awardees that implemented the AHC Model.
