This report summarizes the contributions of Collaboration for Community Health funded partners since its launch in 2018 through 2021. The report combines semi-annual quantitative reporting, qualitative narrative reporting, interviews with funded partners, and administrative data. The report describes the Collaboration’s progress in reaching communities that have historically been denied equitable access to services. It also explores how funded partners have removed obstacles to health and well-being through creating and expanding programs, coordinating across sectors, and advancing policies that promote children’s health and well-being.
